Massachusetts Health Form NEW Requirements

In accordance with Massachusetts Recreational Camp Regulations (105 CMR 430.151), all campers are required to submit a current physical examination and immunization record dated within the past 18 months prior to the start of their camp session.

The health form must be signed by a licensed healthcare provider and must include:

• a statement confirming that the camper is cleared to participate in all physical activities, including sports and full athletic play.

​All submitted health forms will be reviewed by the Hooptown Health Care Supervisor and the Andover Health Department prior to the start of camp. Campers must be fully up to date on all required Massachusetts immunizations. If any form does not meet state or camp requirements, families will be notified. We strongly encourage families to review these requirements with their child’s healthcare provider before uploading forms to ensure timely approval and prevent delays in participation.

​Campers cannot attend camp without a Physician’s Report and Immunization Records on file. Hooptown must complywith the regulations of the Massachusetts Department of Public Health and be licensed by the Andover Board of Health. In order to meet these requirements, a copy of your child’s most recent Physician’s Report and Immunization Records  (dated & signed within 18 months of  camp)

​Insurance

Every camper must be covered by family or accident and health care.

Parents are encouraged to consult with their insurance company to insure campers have adequate coverage.
