Registration

Just a few reminders before you register:

Space is limited so please plan accordingly.

To complete medical information you will need your child’s doctor, dentist and medical information available.

All campers must have a Physician’s Report with Immunization Records on file. Hooptown must comply with the regulations of the Massachusetts Department of Public Health and be licensed by the Andover Board of Health. In order to meet these requirements, a copy of your child’s most recent Physician’s Report with ImmunizationRecords (dated & signed within 18 months of camp)

FORMS MUST BE SUBMITTED BY MAY 1, 2023 – Forms can be uploaded to your campers portal during registration or emailed to coachfaz@hooptowncamp.com

Our Health Care Supervisor cannot dispense any medication sent from home, prescription or non prescription, to a child without the following: A signed and completed Authorization to Administer Medication Form  *The name of the medicine, dosage, dates and time of day that medicine is to be given should be clearly indicated. All medications (prescription and over the counter) must be in its original container.  Prescription medication must have the original pharmacy label.

Authorization to Administer Medications Form can be uploaded during registration or brought on the first day of camp with medication.

Payment – After completing registration you will have the option to pay by credit card or e-check. Gift Certificate or Camp Credit on file – Please contact us for a code to use at checkout.
