Hooptown is looking for energetic and enthusiastic coaches to be a part of our Summer
2025 staff.
If you are an alumni of Hooptown or have a passion for coaching and feel that you will be a
great fit for our staff we invite you to fill out and complete our coaches application.

-Request for Taxpayer Certification
-CORI – Criminal Offender Registry Information Release Form
-SORI – Sexual Offender Registry Information Release Form
-1099 Tax Form
-A Copy of your Photo ID ( Drivers License, Passport, Photo ID, Military ID, Student ID or copy
of your Birth Certificate)
–Heads Up online Concussion Training Certificate​ – All staff must complete the online
concussion training yearly.
-All staff must attend a mandatory Orientation meeting.
